What records are required for my child to enroll in the BCPS system?

All student's must have the following records on file with their school within 10 days of enrolling.

    Birth Certificate

    Current immunization record

    Eye Exam

    Physical Exam

Is the hospital birth certificate okay?

Sorry, but no.  Kentucky requires a state copy of your child's birth certificate.

I don't have a state copy of the birth certificate, where can I get one?

You may request an application from your school's health records coordinator for a Kentucky State Birth Certificate or visit the states website at http://chfs.ky.gov/dph/vital/  or click here Birth Certificate link.  If your looking for an out of state contact for a birth certificate, check the link on the left of that page  or click here Vital Statistic addresses and phone numbers for other states
